Diagnosis or cause of accident
Dower had been employed in the work of connecting pipes for a dump at the 100ft level. He stepped on to a bucket, intending to go to the 200ft level to take a spanner down, and was in the act of signalling to the engine driver, when the "monkey" suddenly dropped, evidently striking Dower after carrying away the stop on the rope. Dower was at the 100ft level at the time that the mishap carried him to 200ft, at which depth he struck the water forcibly. He died a few days later.
