Mathew Harris, 25, died from severe head injuries on February 16, 2006, two days after two metal drill rods weighing more than 500kg and protruding from a parked vehicle smashed through the driver's side of his windscreen at the site. An investigation found his visibility was impaired and there were no beacons or warning flags attached to the drill rods despite company policy.

Mathew Harris is remembered on February 14 every year as part of our Vital Behaviours Remembrance Days. Mathew was driving a tray-back truck to the drilling workshop area to pick up some drill rods whilst another truck was parked in the workshop area with rods protruding several metres from the rear of its tray. Mathew’s truck collided with the rear of the other vehicle and the rods speared through the windscreen, striking Mathew and causing severe head injuries. Mathew died from his injuries two days later, leaving behind a family and people who loved him. Protrusions are just one of many hazards that if not identified and rectified could have devastating consequences. If it doesn’t look right speak up because your life, the life of your workmates and the things we care about are too important not to.
