Diagnosis or cause of accident
On Friday, August 28th, a sad fatality occurred at the Golden Horseshoe Estates mine, Denis O'Brien, a miner, being the victim. It appears that deceased and a miner named Michael Hefron were assisting Alfred Jose, who had met with an accident, to the shaft. O'Brien stepped on a plank when crossing a winze. The plank broke and he was precipitated to the bottom, where he received very serious injuries, from which he died before reaching the surface. The deceased was a brother of the late Mick E O'Brien, the famous Victorian jockey. The funeral was very largely attended.

Probate:- Denis O'Brien, late of Boulder, miner, to Peter Perfeld, £89 16/4; (sealed from South Australia). Kalgoorlie Western Argus Tue 6 Oct 1914. He was the brother of Mick O'Brien a jockey who twice won the Melbourne Cup, on Sheet Anchor in 1885 and Mentor in 1888.
